Most drug labels base their renal dose adjustments on Cockcroft–Gault clearance, so pharmacists still use it for dosing.

Which weight should I use?

A common rule is actual weight if below ideal, ideal weight up to 120% of ideal and adjusted weight above that. Follow your local guidance.
