How to use Maintenance Dose Calculator

  1. Enter clearance in L/h or mL/min.
  2. Enter the target concentration and dosing interval.
  3. Add bioavailability and salt factor, then read the dose per interval.

Worked example

A clearance of 4.5 L/h and a target of 10 mg/L need 540 mg every 12 hours (45 mg/h).

What does steady state mean?

The level at which the amount given per interval equals the amount eliminated, reached after about 4–5 half-lives.

Does this give peak and trough levels?

No. It targets the average level. Peaks and troughs also depend on the half-life and the dosing interval.
