Maintenance dose = CL × Css,avg × τ ÷ (F × S). Peak and trough levels depend on the half-life as well, so monitor drug levels where recommended. For education and clinical decision support only. Results do not replace professional judgment, local protocols or the product information, so always verify doses and values before acting on them.

How to use the Maintenance Dose Calculator

Maintenance dose from clearance, target concentration and dosing interval.

  1. 1 Enter clearance in L/h or mL/min.
  2. 2 Enter the target concentration and dosing interval.
  3. 3 Add bioavailability and salt factor, then read the dose per interval.

Example and practical tips

A clearance of 4.5 L/h and a target of 10 mg/L need 540 mg every 12 hours (45 mg/h).

Frequently asked questions

What does steady state mean?

The level at which the amount given per interval equals the amount eliminated, reached after about 4–5 half-lives.

Does this give peak and trough levels?

No. It targets the average level. Peaks and troughs also depend on the half-life and the dosing interval.
